Foster Parent Application


A foster home is a temporary home for a dog or cat until our rescue is able to find a permanent home.  Thank you for offering to become a foster parent for our rescue organization. To be considered as a foster home you must:

  • Be at least 21 years of age or older.
  • Have consent from your landlord to have a dog or cat if you are a renter.
  • Have all dogs and cats in the foster home spayed or neutered, if they are 6 months or older unless there are medical reasons for not doing so.
  • Have all dogs and cats in the home up to date on annual vaccinations.
  • Agree to have a home visit performed by one of our volunteers. The volunteer will need to see the areas of the home where the dog and cat will be allowed and also to visit with family members and current pets.
  • Be willing to attend any adoption events for which your foster dog or cat is scheduled.
  • Be willing to work with the dog or cat to help prepare it for adoption (e.g., basic training, socialization, housebreaking, etc.)
  • Agree to actively partner in helping place your foster dog or cat and make it available to prospective adopters.
  • Have the foster dog wear a Forgotten Tails Rescue ID tag on its collar at all times when in public or outdoors.
  • Agree to never allow the foster dog to be off leash when it is outdoors unless it is in a securely fenced in area.
  • Agree to never allow the foster dog to be left unattended with young children.

Filling out this application does not guarantee that the dog or cat you choose will be available to you.  We place dogs and cats based on criteria which is in the best interest of the pet.

After completing the Foster Parent Application, please review and complete of our Foster Parent Guidelines and Release of Liability.  The release must be signed before Forgotten Tails Rescue will place a dog or cat for fostering.

All of the above information is true and complete.  I agree to follow all the rules and procedures of Forgotten Tails Rescue (FTR).  I understand that I will decide whether or not to foster any particular animal.  I will not hold FTR responsible for any damage, injury, or harm caused directly or indirectly to any person or property by any foster animal.

I assume the risks of being bitten, scratched, injured, made ill, or frightened by dogs, puppies, cats, and/or kittens in connection with my volunteer work for FTR.  I hereby agree to hold harmless FTR, its individual members, and any venue at which FTR may hold an adoption event, or fund raising event from any and all known or unknown, anticipated or unanticipated, suspected or unsuspected while I am fostering and/or volunteering for FTR.

If I fail to abide by the terms of this Application & Waiver or if I am otherwise unable to meet the requirements of the foster and/or volunteer program, which are subject to change from time to time, I understand that I will be terminated from the program.  I also understand that I may at any time be removed from my position as a foster and/or volunteer by action of the board of directors, and that I will be notified in writing or email should that occur.
